After Finishing Dieting, Hormonal Changes May Lead to Weight Gain?

The answer is YES, according to a new study.

By some estimates, as many as 80% of overweight people who manage to slim down noticeably after a diet gain some or all of the weight back within one year.

A shortage of willpower may not be the only reason for this rebound weight gain. According to a new study in the New England Journal of Medicine, hunger-related hormones disrupted by dieting and weight loss can remain at altered levels for at least a year, fueling a heartier-than-normal appetite and thwarting the best intentions of dieters.

“Maintaining weight loss may be more difficult than losing weight,” says lead researcher Joseph Proietto, Ph.D., a professor of medicine at the University of Melbourne’s Heidelberg Repatriation Hospital, in Victoria, Australia. “This may be due to biological changes rather than [a] voluntary return to old habits.”

Scientists have known for years that hormones found in the gut, pancreas, and fatty tissue strongly influence body weight and processes such as hunger and calorie burning. And the reverse is also true: A drop in body fat percentage, for instance, causes a decrease in the levels of certain hormones (such as leptin, which signals to your brain when you’re full) and an increase in others (such as ghrelin, which stimulates hunger).

What wasn’t so well known, until now, was whether these changes in hormone levels persist after an individual loses weight. To find out, Proietto and his colleagues put 50 overweight or obese men and women on a very low-calorie diet for 10 weeks, then tracked their hormone levels for one year.

So, how do you maintain your weight loss?

Or, is being obese inevitable?

That’s not to say that weight regain is inevitable, or that these drives can’t be overcome through willpower. Although the hormone changes noted in the study are very real physical effects, Proietto says, personality and psychological factors may play a role in an individual’s ability to manage chronic hunger.

“This may explain why some people maintain weight loss for longer than others,” he says. “Maintenance of weight loss requires continued vigilance and conscious effort to resist hunger.”

Promising research is being done to discover ways to restore hormone levels in people who lose weight, Burant says. Preliminary studies from Columbia University, for example, have found that when dieters are injected with replacement leptin hormones, it’s easier for them to maintain or continue weight loss.

“When diabetics don’t have enough insulin in their bodies, we give them back insulin in order to maintain their blood glucose,” Burant says. Researchers should be finding a way to do the same for people who have lost weight, he adds, “whether it’s with a drug, a dietary supplement, or certain nutrients—something that will stimulate the release of these hormones.”

Proietto agrees that finding an appetite suppressant of this sort is the next logical step in hormone and obesity research. Until then, he says, weight-loss surgery is a possible option for some severely obese people who have not been able to keep weight off by other methods.

For now, I will continue to monitor my diet and exercise regularly. Perhaps there will be a vaccine to help the severely obese, and certainly that is a better option than surgery.

Vitamin E Supplements May Increase Risk for Prostate Cancer?

YES, according to a new study.

Men who take a daily vitamin E supplement — a regimen once thought to reduce cancer risk — face an increased risk of prostate cancer, according to results of a large national study.

The finding comes from a report summarizing the latest results of the Selenium and Vitamin E Cancer Prevention Trial (SELECT). Eric Klein, M.D., chair of the Glickman Urological and Kidney Institute at Cleveland Clinic, is the lead author.

SELECT began in 2001 to test earlier research suggesting selenium and vitamin E supplements may reduce the risk of developing certain cancers. Some vitamin supplements containing enhanced levels of selenium and vitamin E were marketed to consumers during this time period with claims of reducing cancer risk.

The paper, which will appear in the October 12 edition of the Journal of the American Medical Association, found that a group of men taking a daily dose of 400 IU of vitamin E from 2001 to 2008 had 17 percent more cases of prostate cancer than men who took a placebo.

“For the typical man, there appears to be no benefit in taking vitamin E, and in fact, there may be some harm,” said Dr. Klein, an internationally renowned prostate cancer expert who served as the national study coordinator.

Prostate cancer is the most common non-skin cancer in American men with a current lifetime risk of 16 percent. For 2011 it is estimated 240,000 new cases and 33,000 deaths will result in the U.S.

Well, I am not taking a vitamin E supplement and will not start now. I am taking a vitamin D supplement, but will discuss continuing this with my physician at my annual physical  exam next month.

Poll Watch: More Americans a Normal Weight Than Overweight

According to the latest Gallup Poll.

The percentage of Americans who are overweight or obese declined slightly in the third quarter of 2011, while the percentage who are a normal weight increased. For the first time in more than three years, more Americans are a normal weight (36.6%) than are overweight (35.8%).

The overweight and normal weight trend lines have tracked closely together since Gallup and Healthways started monitoring Americans’ weight situation daily in January 2008. Though it is noteworthy that more adults are now a normal weight than overweight, it is too early to tell if this shift is temporary or permanent.

The recent slight decline in the nation’s obesity rate is a positive contrast to the rising levels found in 2009 and throughout most of 2010. However, the majority of Americans are still at an unhealthy weight — either overweight or obese (61.6%).

The Gallup-Healthways Well-Being Index uses respondents’ self-reports of their height and weight to calculate body mass index (BMI) scores. Individual BMI values of 30 or higher are classified as “obese,” 25.0 to 29.9 are “overweight,” 18.5 to 24.9 are “normal weight,” and 18.4 or less are “underweight.”

Good news, but there is still more weight to lose.

Obesity Rates Down Across Demographic Groups

Black Americans, the middle-aged, and those with low incomes remain the most likely to be obese across major demographic subgroups in an aggregate of the data collected in all three quarters of 2011 — as generally has been the case historically.

However, obesity rates for each of those groups and all others included in this analysis are either trending down or are statistically unchanged so far in 2011.

So, what does this all mean?

Although a majority of Americans remain overweight or obese, it is a good sign that the obesity rates are trending downward. Perhaps Americans are learning more about adopting a healthy diet and exercise program. Perhaps it is the economic recession and folks are eating out less at fast-food restaurants that have menus that are high in caloric and fat content?

It is hard to say.

But, let’s hope that the trend continues in a downward direction = for better health.

Is There an Association Between Body Mass Index and Periodontal Disease?

Yes, according to a new study.

This study evaluated the association between body mass index (BMI) and periodontal condition in a population of Brazilian women. A hospital convenience sample of 594 eligible women was recruited from a women’s health reference center of Belo Horizonte, Brazil. Four groups were formed considering BMI levels: BMI normal group (n = 352), overweight (n = 54), obesity level I (n = 48), obesity level II (n = 56), and obesity level III (n = 74). Full-mouth periodontal examination was performed and biological, demographic, and behavioral risk variables were evaluated. Obese and overweight women showed statistically significant differences in bleeding on probing, probing depth and clinical attachment level ≥4 mm, and frequency of periodontitis (p < 0.05) compared to women showing normal BMI. The final multivariate model for the occurrence of periodontitis revealed that obesity groups were significantly associated with periodontitis. In addition, age (25-45), smoking, diabetes, and hypertension remained significantly associated with the occurrence of periodontitis (p < 0.05). Periodontitis was positively associated with obesity, and this association was more evident as obesity levels increases. These findings indicate the need for early diagnosis and the inclusion of periodontal care in health care programs for obese women.

Periodontal disease is looking like another complication related to obesity.

Do Licorice Lollipops Reduce Tooth Decay?

Well, they can reduce the bacteria that lead to dental caries (tooth decay) according to a new study.

Sugar-free lollipops containing licorice root extract significantly reduced the bacteria that causes tooth decay, specifically among pre-school children with high-risk of tooth decay, according to a recent study published in the European Academy of Pediatric Dentistry.

The study, funded by the Research and Data Institute of the affiliated companies of Delta Dental of Michigan, Ohio, Indiana, Tennessee, Kentucky, New Mexico and North Carolina, analyzed 66 preschool students ages 2 to 5 enrolled in a Head Start Program in Lansing, Mich. For three weeks, each student received an orange-flavored, sugarless lollipop for 10 minutes, twice daily.

Researchers said they were motivated to conduct the survey by the high number of children who face dental decay and cavities. According to Jed Jacobson D.D.S., M.S., M.P.H., chief science officer at Delta Dental, dental decay is one of the most common childhood diseases, with more than half of children ages 5 to 17 having had at least one cavity or filling.

“We are working to find simple, effective regimens that will encourage prevention and control of dental disease,” he said.

Study results showed a significant reduction in Streptococcus mutans (S. mutans), the primary bacteria responsible for tooth decay, during the three-week period when the lollipops were being used and lasting for an additional 22 days before beginning to rebound.

If this works, why not?

But, the task may be to get the children to suck on the licorice versus chocolate or other caries causing candy.

The lollipops, manufactured by Dr. John’s Candies of Grand Rapids, Mich., were developed using FDA-approved materials by Dr. Wenyuan Shi, a microbiologist at the University of California – Los Angeles (UCLA), and C3 Jian, Inc., a research and development company in California.

There are approximately 700 types of bacteria in the human mouth. While most are harmless, S. mutans is considered the primary culprit in tooth decay. They live in a biofilm (plaque) that adheres to the teeth, consume sugar and release acid that erodes tooth enamel, causing decay. Regular brushing and flossing, along with dental checkups, can help keep S. mutans and Lactobacillus casei in check.
